We are known to be one of the most kids friendly practice, we are proud to keep offering the same service of a thorough health check, keeping record of your child’s weight and height to share those moments seeing your kids growing and to discover if any health issues will come to surface early, we enjoy watching the community growing and that’s brings a smile on our face.
We now also offer a walk in clinic anytime of the opening hours for sick children.
We have highly trained child friendly nurses available 5 days a week for your child’s required immunisations.

Allergy and Asthma
Autumn ,spring, winter or all year long your allergy and asthma can fluctuate and here at Mundijong Family Practice we understand your needs and we are here to help and will provide you with a written plan to sort it out so you can take it at home, school or work.
With population age has been growing, we understand that looking after the elderly people is a must, with understanding how delicate their physical and mental state is, we offer the help with disabled access, instant INR, no out of pocket cost for anything including driving license assessment and minor surgeries, and comprehensive annual health check, and we have just introduced over 65 year old dementia health assessment.
Acute critical care and minor injuries. With our flexibility and wide range of opening hours we are sure to have one of our doctors to tend you as soon as possible.
As we believe in how allied health can complement our service ,we pride ourselves on having a physiotherapist, dietician and psychologist on board and podiatry to join soon. All the allied health professionals bulk bill their patients under the team care plan participation of the GP mental health care plan.

Western diagnostic Pathology supply a superb bulk billing service for all Medicare rebatable test items in our practice

Management of Personal Health Information
The practice has strict guidelines on confidentiality, patient consent, consent for a 3rd party to obtain information, transferring of health information and access and security of personal health information. It adheres to the The National Privacy Principles Act and the Health Records and Information Act.
Your Doctor or the Practice Nurse will attempt to contact you immediately via contact details supplied. If our attempts at contact are unsuccessful, a letter will be forwarded to your address to contact us urgently for an appointment.
NON URGENT RESULTS
The Nurse will contact you via phone to advise that you need to make an appointment to see your Doctor. If our attempts to contact you are unsuccessful, a letter will be forwarded to your address to contact us for an appointment.
If your results are NORMAL we will Not contact you. You are welcome to phone us to check if your results have been returned to us, but you will have to make an appointment to discuss these results.
At no time are results provided to you over the phone due to confidentiality laws.
Patients are contacted for recalls and reminders. If our attempts at contact are unsuccessful a letter will be forwarded to your address to contact us for an appointment.

The practice is committed to supporting the doctors, nurses and administration staff to participate in on-going education.
DID NOT ATTEND (DNA) AND CANCELLATION POLICY
Due to the high demand for bulk billing appointments and the high number of non-attendances and cancellations for GP appointments, we will charge a fee of $40 per consultation or if a double consult a fee of $80, that is not attended or not cancelled within 4 hours of the appointment time.
If this account is not paid, a letter of suspension of services and the outstanding account will be issued. This letter will see your access to all services suspended until accounts are paid. There may also be the possibility of permanent suspension from the practice at the Doctors’ discretion for repeat offenders.
We have many wasted appointments every day and by introducing this policy we hope patients will consider cancelling their appointments so that others may access them if required.

If you need more time with the doctor, please tell the receptionist when you make your booking. Urgent medical problems will always be dealt with promptly.

Staff at Mundijong Family Practice
Dr Abu Bham (MBChB; FRACGP)
Dr Bham graduated in 2010 from the University of Leicester, UK. He completed his GP training in Gloucester, UK in 2015 and worked as a GP in the same area before moving to Perth in 2017.
He is interested in and passionate about all areas of General Practice, believing in the importance of "patient centred care."
His special interest include chronic disease management, skin checks/skin cancer, men's health, health checks and care of the elderly.
Dr Hassan Zaheer (AMC; MBBS)
Dr Hassan is a general practitioner with special interests in family medicine and mental health. Dr Hassan is able to provide mental health care plans and works evenings and weekends.
Dr Yazan Absi (MD; AMC)
Dr Yazan is a general practitioner who has special interests in neurology, chronic disease management (such as diabetes mellitus, hypertension etc.), chronic disease care plans and sports injuries.
Dr Amgad Agaiby (MBBS)
Dr Amgad is a general practitioner who has special interests in cardiology and mental health.
Dr Amgad is available Thursdays and every second Saturday.
Mr Sam Pierce (BSc (Physio))
Sam is a qualified physiotherapist with specialties in foot and lower limb injuries as well as treating back, neck and shoulder pain.
He also offers dry needling/acupuncture, trigger point therapy, joint mobilisation and manipulation, taping, exercise and gym programmes and gait analysis with CMV analysis.
Sam is available for appointments on Wednesdays.
Miss Farai Makwiranzou (BSc Podiatry)
Farai is a qualified podiatrist with additional studies in diabetes and is pursuing her masters in public health. Farai’s interests include all conditions related to the foot and ankle. Including, skin conditions of the foot, heel pain, orthotics and diabetes foot care. Additionally, she offers foot care services to the elderly in their homes, particularly those with dementia related conditions.
Dr Charmaine Lim (BSc (Chiropractic))
Dr Charmaine is a qualified chiropractor with a holistic style of care. She is able to assess your whole body to pinpoint the cause of your problems, rather than simply focusing on your symptoms.
Charmaine’s special interests are family care, sports, rehabilitation and posture, she also provides dry needling.
Dr. Lim is available for appointments on Tuesdays
Ms Farzana Pithawalla (BA (Psychology))
Farzana is an experienced registered psychologist and an associate member of the Australian Psychological Society. Farzana has worked extensively with high risk populations with suicidal ideation and self-harm behaviours. Farzana also has experience working with anxiety, depression, alcohol and other drug issues, anger, chronic pain, sleep, trauma, post-natal depression, behavioural issues, trauma, personality disorders, grief and loss issues among many others.

Chantal is our valued practice nurse who has a special interest in wound care.
